Information Needs

DataMed requires information on the patients and results from testing labs. DataMed
requires information on the patient’s contact information, language, age, allergies, medication, current and past health record, education, work experience,  and insurance coverage. This data is gathered from an initial form that the patient fills out during their first visit at the clinic, and updated every 4 months or through prompted questions during every visit.

The patient fills out a Medical History Assessment (Appendix 2), that assesses the patient’s current and past health state in yes-no question format with an explanation if necessary. As well as a brief description of the most recent health or medical issue. Each patient is also recommended to do a full body physical exam, but can be deferred upon preference. This will provide an overview of the state of the patient and their potential needs, visit purposes, and frequency. Each patient is assigned a single folder through the electronic health record to record interactions and transactions with DataMed. This information will be for the Doctor to better understand the patient and be used as a reference going forward.

DataMed also requires a preferred time of contact, time of appointment, and annual check up date. The patient fills out a Preference Form (Appendix 3) which will help determine scheduling and appointment booking for the administration team and for the public relations and marketing director to utilize this information in creating a suitable campaign.
