Application Package

Dental hygienists require the theoretical knowledge and clinical experience to perform the ADPIE process of care. Each aspect of the process of care; assessment, diagnosis, treatment planning, implementation and evaluation are integral components of optimal patient care. As I was considering the type of clinical environment for my job search, I found a job posting for a registered dental hygienist with Dr. Mindy Pho at Heritage Periodontics. Choosing to focus my application package on this specialty is very desirable since I would like to gain experience in a periodontal practice treating patients with extensive periodontal disease.

This job application contains four documents: a PDF copy of Heritage Periodontics job posting for a dental hygienist, my cover letter to Dr. Pho,  my resume, and three written requests for references.   Please click on each of the links below to access the PDF files of this application.
