Subject: Reimbursement for Eye Doctor Appointment

 

Dear Student Services Representative,

 

During my enrolment at McMaster University, my student fees included a Vision Plan that covered up to $50 of the cost of an eye exam. When I received my eye exam, I was charged the full cost. The optometrist specified instructions to visit the financial aid office to send my claim receipt, but this occurred during the first Covid-19 lock down. I was not able to promptly get in touch with the office at the time, and it slipped my mind later in the term. I am writing to request reimbursement post-enrolment.

The exam was performed at Campus Optical in March 2020 and totalled $90. I have enclosed the receipt below.

Though I am aware that my coverage was specified for the winter 2020 semester, the receipt is dated back to when I first paid for the services. I am a McMaster alumnus who has always appreciated the generosity and flexibility that McMaster’s student services have offered.

Please let me know what the next possible steps are for reimbursement.

Dear Ms. Bhatt,

 

Thank you for reaching out about your eye appointment at Campus Optical. We are very sorry to hear that you were not appropriately reimbursed. You are correct that expenses up to $50 at Campus Optical are eligible for coverage under the student health plan.

Unfortunately, though we are sympathetic to the complications caused by Covid-19, our coverage plan works in accordance with the policies of Sun Life Assurance. All claims must be received no later than 90 days after the end of the policy year in which the claims were incurred or 90 days after the end of your coverage, whichever is sooner. As your claims were incurred beyond the 90 day threshold, we are regrettably no longer able to reimburse you for the cost.

If you are still interested in pursuing the claim or have further questions, you can contact Sun Life Assurance directly at 1-800-361-6212. Their availability is Monday Friday, 8 a.m. 8 p.m. ET.
