Initial Visit and Consultation with Vitality Fitness Club

Here are some sample questions you’ll be asked prior to starting a program with us:

Medical Clearance:

Has your doctor ever said that you have a heart condition and should not perform physical activity?”–a full medical clearance is covered when conducting our health assessment and orders you to fill out a Physical Activity Readiness Questionnaire (PAR-Q).

If any questions are answered “yes” on the Par-Q Form , please also fill get your Family Physician to fill the ParMed-X Form and bring it upon first consultation. 

Motivation for initiating a fitness program:

Describe a time when you were at your fittest, Who was important in your life at that time?”

“How will exercise impact your life? What if you didn’t begin to exercise? How would your life look in 5, 10, 15 years?”

“What is your short term health and fitness goal? What is your long term goal?”

On selection of training type and frequency:

What experience do you have with resistance training? with aerobic/endurance training?”

“What are some activities you partake in regularly or want to be able to enjoy?”

“What is your work schedule like? What sort of family, social, or volunteer commitments do you have day to day?”

After completion of your initial assessment, as well as health and fitness assessments, our health and fitness experts will offer you a customized training plan to help you meet your goals.
