It is always about:
1) The patients
2) A systematic process
– Goals
– DRP
– Alternatives
– Plan
– Monitoring – subj, obj, freq, by whom
– Process: DAP, check DI/ RF/ LF
3) Taking initiatives – for your patients
Rotation 2 Emerge
– writing SOAP notes
– finding DRP – list ALL the medical problems – list ALL the meds –> and see if they match
Rotation 3 ICU
1. medical problem –> on the optimal drug therapy?
2. assume responsibility for your patient – advocate for your patient!!!
3. drawing, let your student ask whatever they want
4, physiology/ anatomy/ pharmacology/ PK –> more convincing than evidence at times; tailor to the need of individuals
5. extensive and focused reading – keep record!
Rotation 4 CSICU
1. be efficient – CE, working up patient (go to one reliable source)
2. make your own round if there is none
3. know everyone’s role and see where you could fit in
4. know what you want –
In my future career, I want:

5. Process – clean up MAR – medical problems – anything missing (indicated meds, old meds), right dose/ form/ route?
