{"id":371,"date":"2014-11-28T12:51:14","date_gmt":"2014-11-28T19:51:14","guid":{"rendered":"https:\/\/blogs.ubc.ca\/lfsta\/?page_id=371"},"modified":"2017-06-12T16:59:14","modified_gmt":"2017-06-12T23:59:14","slug":"schedule-b","status":"publish","type":"page","link":"https:\/\/blogs.ubc.ca\/lfsta\/schedule-b\/","title":{"rendered":"Schedule B"},"content":{"rendered":"<p>The <a href=\"https:\/\/blogs.ubc.ca\/lfsta\/files\/2014\/11\/Dues-Deduction-Form.pdf\">Dues Deduction Form<\/a> or Dues Deduction Form authorizes UBC to deduct union dues, initiation fees and any assessments as established by CUPE 2278 from your wages.<\/p>\n<p>It is a mandatory requirement that every TA must complete.<\/p>\n<p>You do not have to complete a Schedule B for EACH of your TA appointments in every given term, but you do have to complete a new Schedule B for each session you will be TA&#8217;ing.<\/p>\n<p>Please note:\u00a0 the Schedule B form requires a <strong>SIGNATURE.\u00a0<\/strong> Please do not type or write your name on the line above &#8220;Signature of Employee&#8221;.<\/p>\n<p>The Manager, Graduate Programs for LFS collects all Schedule B forms for TA&#8217;s who have been appointed up to August 9th and forwards these on behalf of the TA&#8217;s to CUPE 2278.\u00a0 If you are appointed AFTER August 9th during any Winter Session, it is your responsibility to complete this form and send it to the union with a copy to the LFS Manager, Graduate Programs.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The Dues Deduction Form or Dues Deduction Form authorizes UBC to deduct union dues, initiation fees and any assessments as established by CUPE 2278 from your wages.
