Are there any potential risks in participating in this study?

We do not foresee any risks in participating in this study. You are free to quit the study at any time and can choose not to answer any questions that make you feel uncomfortable.

What are the benefits of participating in this study?

Participants who fill out our pre-screening survey will be entered in a prize draw for a 1 in 25 chance of winning a $20 Amazon e-voucher. If you are invited to join this study, you will receive up to an $80 honorarium for participating in the program and surveys. 

Additionally, as a participant in this study, you may benefit from the enjoyment of participating in the study program and your contribution will play a crucial role in building future wellness programs in your community. What if I want to withdraw from the study?

Your participation is entirely voluntary. If you decide to take part in this study, you are free to withdraw at any time without giving any reasons for your decision.

How will my information be kept confidential?

All of the information that you provide as a participant will be kept strictly confidential and will be identified by a code number (not by your name). Your contact information will be stored separately from other survey information and will only be used to inform your eligibility for the study participation and/or the prize draw results.
