TDI Questionnaire

The TDI is available to complete in both an online version and a paper version. Instructions for administering either version are included below.

TDI packages should include one copy of each of the following three documents presented in an unsealed 9 x 12" envelope (so that participants can seal their responses before returning them to the TDI facilitator) labelled "HELP TDI Study" (so that the envelope contents are clear once they have been completed and returned):

The TDI Parent/Caregiver Consent Letter contains all study information that will allow parents to make an informed decision whether to participate.

Please note that this version of the TDI is for pilot use only and must not be shared outside of the study. If you would like to have a copy of the TDI for purposes outside of the TDI pilot data collection, please contact the TDI Research Project Coordinator for a watermarked version.

As the TDI project is currently being implemented in its pilot phase, parents/caregivers are encouraged to provide feedback on their experience filling out the questionnaire as well as any comments/concerns/questions that arose related to the TDI or the implementation process.

When the participant returns the TDI package, please ensure the envelope is sealed and write "TDI study" on the front.

Place the sealed envelope in a secure location, such as a private, locked filing cabinet.

When you are ready to return the completed TDI packages to HELP, please place them in another larger envelope so that responses are double-sealed and mark the envelope "Confidential".

Please mail the entire package back to the Human Early Learning Partnership ensuring you request a tracking number for the package.

We value participants’ time! Participants who complete the TDI can choose to be entered into a draw for a $50 gift certificate.

For those who complete the paper survey, TDI facilitators are asked to gather the participant’s ballot (name and email address). We have $50 for a gift certificate for each community to facilitate a local draw. How you decide to facilitate the community draw is up to you.
