    • This template may serve as a useful reminder for parents/care givers to take home with them if they cannot complete the TDI on site. Additionally, these bookmarks may be left for distribution in a waiting room or at a front desk. Please note this version is a template, so may be edited to reflect the contact information or drop-in details specific to your site. The bookmark is formatted to be printed off 8.5in x 2.5in.
  • TDI Simple Poster Template (English) (Chinese)

    • The flyer is meant to catch the attention of clients and alert them to the TDI study. Please note this version is a template, so it should be edited to reflect the name and/or contact information of a TDI facilitator(s) at your site.

    • This information sheet can be distributed to participants who are interested in learning more about the study or posted as a flyer to alert clients’ attention to the TDI study.
