Before you begin sharing the TDI with families, please complete these 5 simple steps:
1. Watch the TDI Facilitator Training
If you do not have the opportunity to attend an in-person or webinar training session, please be sure to watch the video recording of the TDI Facilitator Training.

2. Review the TDI Questionnaire
Review a copy of the TDI to familiarize yourself with the questions asked. Click here to view the paper or online TDI.
3. Read the Facilitator Consent Letter
Read the Facilitator Consent Letter, which outlines the details of the TDI pilot implementation project and the responsibilities of the TDI facilitator.
4. Complete the Data Confidentiality Agreement
Complete the Data Confidentiality Agreement, which summarizes the TDI facilitator's role in helping to protect the privacy of participants involved in the TDI project.
5. Check TDI Site Permission
Check that your agency/program is listed as a confirmed TDI distribution site.
