    Hello Priyanka,

Thank you for posting your Progress Report. There are a few of issues. First, your reader(s) need to have the authority to act on the final recommendations – and the assignment instructions request that you name your reader. Please do clarify this for me.

Next, the survey needs a correct ethical introduction, please do review the lesson instructions and the example on the Instructor’s blog and compose a proper introduction.

Next, some questions are too personal.
You need to rephrase these questions so that they are general and not personal.

For example: these questions are too personal:

Do you receive any recognition of your role as a dental hygienist?
Do you receive any recognition of your role as a dental hygienist?
Are your clients able to identify your role as a dental hygienist from dentist and dental assistant in the office?
Does your dentist appreciate your role and partner up to help with prevention?

You want to avoid questions that might lead the respondent to reflect on their personal situations in context with other people. I hope you understand

Please make these changes and alert me with an email with a hyperlink to the page. Thank you.
